Step-by-Step IVF Process: A Complete Timeline from Testing to Transfer

It is natural to feel a mix of hope and apprehension before starting In Vitro Fertilization (IVF). My patients often tell me their biggest fear isn’t the injections themselves, but the unknown. “How do I know if my eggs are healthy enough? Will the retrieval hurt? What happens inside the lab?”

In my +30 years as an infertility specialist, I have learned that knowledge is the best antidote to anxiety. Below, I break down the Step-by-Step IVF Process, removing the mystery and showing you exactly how we turn medical science into the miracle of life.

1. Pre-IVF Testing and Preparation

Before we prescribe a single medication, we must understand the unique environment of your body. Think of this as drawing the architectural blueprint before building a house.

We perform comprehensive Pre-IVF Testing, which includes checking your Ovarian Reserve (AMH levels, Antral Follicle Count) and conducting a semen analysis. This defines your starting point.

2. Stimulation of the Ovaries

Once your treatment plan is set, the active phase begins. Naturally, a woman releases one mature egg per month. In IVF, our goal is to safely encourage the ovaries to mature multiple eggs simultaneously.

You will self-administer hormonal injections (like gonadotropins) for about 8 to 12 days. Patients are often terrified of the needles, but they are incredibly thin—much like a diabetic insulin pen. During this phase, you will visit our clinic frequently for transvaginal ultrasounds and blood tests. We monitor the growth of your follicles (the tiny fluid-filled sacs that contain the eggs) to prevent a rare complication called Ovarian Hyperstimulation Syndrome (OHSS).

3. The Egg Retrieval Procedure

When your follicles reach the optimal size (usually 17-20mm), you will take a final “trigger shot” to induce final maturation. Exactly 35-36 hours later, we perform the Egg Retrieval.

Does egg retrieval hurt? No. In my clinic, you will be under mild, short-acting intravenous sedation. You will sleep for about 15-20 minutes, feel absolutely nothing, and wake up feeling like you just had a deep nap. I use an ultrasound-guided needle that passes through the vaginal wall directly into the ovaries to gently aspirate the fluid inside the follicles, collecting the eggs.

4. Fertilization in the Laboratory (IVF & ICSI)

Once the eggs are retrieved, they are immediately handed to our embryologists. In a pristine laboratory environment, we combine the eggs with the partner’s (or donor’s) sperm.

If there are severe male factor infertility issues, we don’t leave fertilization to chance. Instead, we use Intracytoplasmic Sperm Injection (ICSI). Under a high-powered microscope, a single, healthy sperm is selected and injected directly into the center of the egg. When I first brought the ICSI technique to Turkey in 1994, it revolutionized our ability to help couples who previously had zero chance of conception.

Over the next 3 to 5 days, the fertilized eggs (now embryos) mature in incubators that mimic the human body. We monitor them closely to see which ones grow into robust blastocysts.

5. The Embryo Transfer

This is often the most emotional day of the entire journey. The Embryo Transfer is a simple, painless procedure similar to a routine pap smear—no anesthesia required.

Using a soft, flexible catheter, we gently guide the chosen embryo through the cervix and place it in the ideal spot within the uterine lining. You can actually watch the procedure live on an ultrasound monitor. It takes about 10 minutes.

IVF Success Rates: What Science Says

Your chances of success are heavily influenced by maternal age and egg quality. According to data from the American Society for Reproductive Medicine (ASRM) and the Society for Assisted Reproductive Technology (SART), live birth rates predictably decline as a woman gets older, which is why early consultation is critical.
